Freedom Plaza is a unique community featuring carefree living with a wide array of beautifully appointed residential choices. Maintenance Free Residences
Residents at Freedom Plaza enjoy independence and autonomy – and control over the structure and delivery of services and care. They have a wide variety of activities to choose from, including programs to encourage wellness, social engagement, and ongoing education. As health and care needs evolve, residents can access the whole menu of senior care services at little to no increase to their monthly fee.
Freedom Plaza is situated on a 140-acre landscaped campus with a private lake, walking trails, and an 18-hole executive golf course and putting green. The community features over 400 independent living residences, including studios and one- and two-bedroom apartment residences. The campus also offers 83 assisted living residences; 113-bed skilled nursing, 12 private rehabilitation suites; and 10 deluxe memory care apartments.
Senior adults can move to independent living residences and be assured that if they ever need additional care, they are still at home.
Assisted Living at Freedom Plaza
The administration at Freedom Plaza collaborates with families to develop an individualized plan for every resident — including grooming, bathing, dressing, medication management, and other ADLs. One monthly fee includes all community services and amenities, including fitness classes, restaurant-style dining, and an on-premises health care center. The facility is also equipped to offer respite care, giving caregivers a much-needed respite. (Reservations are required for respite stays.)

Memory Care Services
Freedom Plaza was recognized as a 2022-2023 Best Memory Care community by U.S. News & World Report. We provide a safe and comfortable environment specially designed for those with dementia, including Alzheimer’s disease. The staff provides life-affirming activities and personalized care that coincide with each individual’s stage of memory loss. The innovative Heartfelt CONNECTIONS – A Memory Care Program® provides those with cognitive impairment the respect, dignity, care, and quality of life they deserve.
Heartfelt CONNECTIONS is a nationally recognized, individual-centered, activity-centric program. The program works with loved ones to design an individual care plan specific to each resident’s family, friends, career, hobbies, and preferences. Each resident’s unique abilities are highlighted, resulting in more enriching lives. In addition to standard care and assistance with ADLs, Freedom Plaza offers:
- Dementia-specific programming, including art, music, and pet therapy
- Access to community amenities such as the salon and barbershop, fitness center, and pools as deemed appropriate by caregivers
- Small-cluster group activities
- Community excursions
Freedom Plaza’s Memory Care Program promotes a sense of pride and accomplishment in daily activities, encourages eating and good nutrition, supports self-worth and self-esteem, and works to minimize fear, anxiety, and frustration.
Skilled Nursing and Rehabilitation
Should an assisted living or memory care resident’s needs progress to needing skilled nursing, they can access the help they need through our skilled nursing program. The program was awarded the Joint Commission’s National Quality Seal of Approval and offers highly personalized senior care and professional transition support. Staff provides compassionate assistance with activities of daily living, as well as ensuring every resident still engages in appropriate activities to maintain social contact and relationships.
For those who need short-term care after an illness, injury, or hospital stay, the skilled rehabilitation team members will work to create a customized short-term care plan. Like our other programs, the rehabilitation program at Freedom Plaza is award-winning, garnering the 2022-2023 Best Nursing Home Short-Term Rehabilitation Award from U.S. News & World Report.
